Safety violation might have caused fatal fall at plastics factory

Employees in industrial facilities in Ohio and elsewhere typically face numerous dangers in their workplace environments. Employers must know that every safety violation can potentially cause serious workplace injuries or worse. A plastics manufacturing facility in another state will be the subject of a third investigation in seven years after the recent death of an employee.

According to the sheriff's office, an emergency call came in at about 9 a.m. on a recent Wednesday. Reportedly, a co-worker came across the deceased worker. Emergency workers rushed to the factory to find the body of a 55-year-old employee in a crusher machine. They said the machine was not active when the worker fell into it, but they found him buried under heavy plastic piping. He was already dead when paramedics arrived.

The Occupational Safety and Health Administration will likely investigate this fatality to determine the circumstances that led to the fatal fall. The sheriff's office will assist with the investigation. OSHA records show that this company received multiple citations for safety violations in 2009 and 2013.

Investigators will determine whether this fatality resulted from a safety violation. If so, sanctions may be imposed on the employer. The deceased worker's surviving family members can seek assistance to help with the unanticipated economic losses that were brought about by the tragedy. They can file benefits claims with the workers' compensation insurance fund of Ohio. Along with coverage of end-of-life expenses, the family may also receive a financial package to make up for lost wages, and help with day-to-day living expenses.
